Chosen Scenario: Sepsis You are a third-year student nurse working in the High Dependency Unit. You have just come in for your shift and have been given a patient handover. Situation: Barbara Davis is a 48-year-old American woman who was admitted to the hospital with a 36-hour history of progressively worsening abdominal pain, nausea, and vomiting. Background: Barbara has a past medical history of hypertension, type 2 diabetes mellitus and obesity, and takes regular medications for these conditions. Five days ago, she was admitted to the ICU and treated for acute pancreatitis. During the transfer to the hospital, paramedics inserted a peripheral intravenous catheter (PVC) in her right antecubital fossa to begin an infusion of normal saline at the maximum flow rate. Her HR is 140 beats/min (regular, sinus tachycardia), BP is 95/55 mmHg, CRT is 4 seconds, and her Temp is 39.2°C. Her peripheries are cool, her skin is mottled, and she is oliguric. Her BGL is 8.5mmol/L and she appears confused. When you remove the heavy tape and dressing from the old PVC site, you notice that there is some pus around the area, it looks red and is tender to touch.
